The following form is to be completed by the client, or representative of the client, who, by filling out this form, agrees they (client) currently hold full ownership/guardianship of the animal and/or rights/permission to move and board the animal/place the animal in another’s care.

By signing this form, the client states they have read, understood and agree to the terms and conditions.
